While it can be unpleasant to think about something bad happening to a family member, medical emergencies do happen. For example, if you’re not at home when your elderly relative falls or becomes ill, do you have a way for first responders to safely make entry into your home to provide care? In the event of an emergency, if your family member suffers from Alzheimer’s, dementia, autism, etc., and can’t articulate important safety or health information, we can provide emergency contact and access information to first responders.
Please fill in the following form and submit it.
We will keep your information secure and it will not be shared with any outside entity. It will only be used to gain access to your relative and to contact you in the event of an emergency at your home.
If you have any questions, please email the Communications Department.
